Design Review Request Form
The prior approval of the Design Review Committee shall be required for the construction or alteration of any improvement upon any Lot that already contains an occupied residence or is used in conjunction with a Lot that already contains an occupied residence located within Oakdale Park.
Step 1: Provide Contact Information (all fields required)
Step 2: Write a description of the improvements to be made, detailing the complete scope of work. If upgrading (i.e., patio), point out any changes from what currently exists (i.e., "CHANGE: patio will be extended out 8 feet.")
Step 3: Email required supporting documents to [email protected].

- Images: aerial view of home (i.e., GoogleMaps) showing area to be improved plus pictures specifically of improvement area.
- Painting requests must include an image of the front of your house with the homes on either side and paint chips / stain samples or website links to colors (please indicate body, trim, accent colors).
- If applicable, please include a plot plan with dimensions, elevations and relation to home, fence, property lines – please provide detailed measurements
- Materials Information: details of materials to be used, may include warranty information, product descriptions (i.e., link to manufacturer website), image of product color and style, construction materials to be used, etc.
Step 4: You submit this form with the understanding of each of the following statements:
- I have reviewed the Design Guidelines for this project on www.oakdaleparkstw.com - Bylaws and Declaration of Covenants, Conditions, and Restrictions.
- This form includes a detailed description and I will email attachments and relevant supporting images, photographs, plot maps, materials and landscaping information.
- I understand that I am responsible for obtaining any permits and approvals required by the city of Stillwater and/or Payne county.
